text form, is available here. If you use this
application be sure to print it out and complete it by hand-- it is not suited to
completion onscreen. You may then save and print the result for your signature.
When you have completed and signed your
application, please fax it to the fax number available at or send
by postal service to the mailing address. Mail is
best if you have no means of sending the required two photographs
electronically. Very important: if you send anything by fax or mail,
be absolutely sure you include your email address. Photos or questions, not the
application, may be sent by email. When we've
received suitable photos and a signed application, we will typically prepare a page for
your review.
